Brilliant lectures

Prof. Susie Lau: Robotic Cytoreduction of Ovarian Cancer

Prof. Susie Lau from McGill University, Canada, firstly by briefed the fundamentals of robotic cytoreduction of ovarian cancer by reviewing of a large number of literature. She noted that the integration of robotic technology into the surgical management of patients with ovarian cancer appears to be feasible for well-triaged patients, and can increase patients’ survival rate and improve perioperative outcomes. Satisfactory tumor regression remains the surgical goal, and future studies should clarify which patients are best suited for minimally invasive tumor reduction surgery. She then shared her experience and tips on the management of robotic intermediate cytoreduction, robotic diaphragmatic resection, and robotic left para-aortic tumor reduction surgery for recurrent cancers in conjunction with several surgical videos. Finally, she hoped that in the future, guidelines for selecting patients for the MIS/robotic platform could be developed to enable better guidance for tumor reduction.

Prof. Isabela Scheibner: The Pelvic Floor Diagnosis in the Urogynaecological Outpatient

Prof. Isabela Scheibner from Aschaffenburg Hospital in Germany firstly introduced the ecological  diagnostic methodology of urinary system: (1) collection of history using a standardized questionnaire; (2) gynecological examination, ancillary investigations, palpation, and ultrasound of the pelvic floor; (3) objectification of urination manifestations with urination recordings; (4) urodynamic examination; (5) diagnosis according to international standards; and (6) entry of results into a database that allows for continuous assessment of outcomes. She then elaborated on performing gynecological examination and ultrasound with an empty or full bladder, as well as urodynamic examination, and analyzed the results. She emphasized the importance of adequate communication with the patient during the consultation, determining the treatment with the patient, keeping accurate and complete records, and agreeing on the schedule of follow-up examinations.

Prof. Marek Jantos: Diagnosis and Treatment of Intractable Chronic Pelvic Pain

Chronic pelvic pain refers to any chronic pain that can cause pain in the area below the umbilicus and above the thigh. Prof. Marek Jantos of the Behavioural Medicine Institute of Australia emphatically introduced the use of pain mapping in the diagnosis and treatment of chronic pelvic pain. He said that pain mapping can localize the site of pain occurrence, quantify the severity of pain, determine the nature of pain (e.g., burning, tingling), can identify the muscular origin of pain, emphasize the role of peripheral mechanisms of pain, guide treatment based on the results of the distribution of the patient's pain, establish a practical pathway to resolve the pain, and provide an objective means of evaluating the effectiveness of interventions. Prof. Marek Jantos' meticulous explanation helped us to have a clearer understanding of the use of pain mapping.

Prof. Hua Keqin: Strategies of Single-Port Laparoscopic Fertility-Sparing Treatment for Obstructive Reproductive Tract Malformations

Prof. Hua Keqin from Obstetrics and Gynecology Hospital of Fudan University firstly led us to review the development history of single-port laparoscopy, introduced the current development and advantages of single-port laparoscopy, and then shared her surgical experience and skills with the data and surgical videos of rare cases of obstructive reproductive tract malformations preserving fertility. The surgeries included single-port laparoscopic residual angle hysterectomy, single-port laparoscopic bilateral uterine fusion, and so on. Prof. Hua and her team's exquisite surgical operations were admired by the participants who expressed that they had gained a lot of benefits. Finally, Prof. Hua summarized the advantages and experience of single-port laparoscopy, which deepened everyone's impression and understanding.

Prof. Liu Kaijiang: The Technical Improvement and Related Problem of Fertility Preserving Surgery for Cervical Cancer

Prof. Liu Kaijiang from Renji Hospital of Shanghai Jiao Tong University School of Medicine firstly introduced the indications and contraindications of fertility preservation surgery for patients with cervical cancer, emphasizing that preoperative evaluation is very crucial and the least easy to evaluate is the absence of infiltration phenomenon above the internal cervical orifice. He then shared the technical points of fertility preservation in patients with cervical cancer and the improvements made by his team, including lymph node dissection, uterine artery preservation, paracentesis, management of the vagina and cervical canal, safe distances between the incision margins of the cervical canal lesions, vaginal-uterine anastomosis, cervical cerclage and timing, and postoperative pregnancy. He stressed that the key to fertility preservation surgery in patients with cervical cancer is the management of the uterine isthmus, which is the key to surgical success, pregnancy outcome and oncologic outcome.

Prof. Xia Zhao: Fine Anatomy and Surgical Techniques of Radical Hysterectomy

Prof. Zhao Xia from West China Second University Hospital of Sichuan University explained the anatomical points of many cervical surgeries with clear pictures of pelvic tissue anatomy, and then demonstrated the key points and techniques of radical hysterectomy with surgical videos, emphasizing the need to avoid organ damage during the operation. Prof. Zhao said that radical hysterectomy should focus on the patient's prognosis and the treatment effect. Pelvic anatomy is of great significance, and Prof. Zhao's wonderful explanation helped us have a deeper impression of the essentials of radical hysterectomy.
